SDMA: The Clinical Evidence for Early Kidney Disease Detection

SDMA (symmetric dimethylarginine) is the cornerstone of early chronic kidney disease (CKD) detection in dogs and cats. This page summarizes the evidence behind its clinical use.

The Biological Basis

SDMA is released into the circulation at a constant rate during protein degradation and cleared almost exclusively by renal filtration. Unlike creatinine, it is not affected by muscle mass — the key to its early-detection advantage.

SDMA rises when kidney function is only modestly reduced — enabling detection when intervention can still slow progression.

The Muscle-Mass Advantage

In cachectic or geriatric patients, creatinine is falsely low (low muscle mass → low creatinine), masking kidney disease. SDMA is muscle-mass-independent, making it the more reliable marker in exactly the patients most at risk.
